What to Bring

When coming to Nacogdoches Memorial Hospital for any service, you will need the following:

1. Doctor’s order (unless reporting directly to the emergency room). This provides the hospital with the necessary medical information required for your care.
2. Picture identification card (driver’s license or a similar form of identification).
3. All current medical insurance cards (Medicare, Medicaid and private insurance cards).
4. Social Security card.
5. If you plan to be admitted to the hospital and have a Directive To Physicians and Family or Surrogates or a Medical Power of Attorney, please bring a copy. If someone has been appointed to make medical decisions on your behalf, please bring your documents. Such as:

  • Letters of Guardianship
  • Medical Power of Attorney
  • Durable Power of Attorney
  • Directives to Physicians
  • Documentation that a surrogate-decision maker has been appointed if you live in a nursing home or home for people with disabilities.
  • The TDPRS or TYC Placement form for children in foster care or residential placement.
